We provide high quality, individualised, interdisciplinary rehabilitation for Doncaster stroke survivors and their families in community settings.
Rehabilitation can be provided by our team for a maximum of six months following a diagnosis of stroke. After this time, we refer patients with ongoing rehabilitation needs to the appropriate service according to the Doncaster agreed stroke pathway.
The amount of visits received will depend on individual needs identified on assessment, and therapy is carried out as agreed with the client around their set personal goals.
Our team comprises a team lead, stroke rehabilitation nurses, physiotherapists, occupational therapists, speech and language therapists, and speech and language assistant, social worker, clinical psychologist, dietitian, rehabilitation assistants and admin support.
Who can use our service
Referrals are usually received by the team from the stroke units at Doncaster Royal Infirmary (Ward 16) or Mexbrough Montagu (Wentworth Ward), although we also receive referrals from GPs, social services or other members of the healthcare team.
