The Doncaster Telehealth Service provides individualised Information, Education, Support and Advice for adults (aged 18 and over) living with long term conditions to encourage independent living within the community and self-management of these condition’s.

The hours of work are Monday to Friday 8.30am – 5pm

The team work over the telephone in partnership with other community services such as the district nurses, respiratory and cardiac and Parkinson’s specialist nurses and GP practices.

The service  is provided by a team of qualified nurses who have a range of skills and experience to support patients in self-management of Long-term conditions.

The Telehealth service also offer telephone support for patients living with a palliative diagnosis.

The Team use a range of technology and skills such as motivational interviewing to enable patients to make lifestyle changes , improve  recognition of symptoms thus empowering the patient

Referrals can be made via the Single Point of Access 01302 566999. Referrals most often come from district nurses and specialist nurses but can come from other professionals, patients or carers.
