Hawthorn and Hazel Wards are nurse and therapy led units. This means that the people responsible for your care are the nurses and therapists. On admission you will be allocated a named nurse and an associate nurse who will coordinate an individual plan of care with you.
Hawthorn and Hazel Ward receive medical cover from a GP from a Doncaster Practice, and should you need to see a doctor while you are here this will be arranged fro you by the nurse on duty. The nursing team will also liaise with your own GP on discharge.
Physiotherapists, occupational therapists, nurse and rehabilitation assistant s will play a big part in your rehabilitation on Hawthorn and Hazel Wards. These are the people who will ensure that your independence is supported and they will ensure that you have everything you need.
Other professionals may also be involved in your care; these include dieticians, speech and language therapists, podiatrists and pharmacists.
All members of staff wear a uniform and name badge.
