The Assertive Outreach Team aims to promote the social inclusion of its service users through developing and maintaining long term supportive and therapeutic relationships with people focussing on the recovery model to maximise their strengths and abilities while acknowledging the limitations and difficulties caused by their disorder.
We work alongside service users and their carers using a psycho-social interventions approach to identify and meet their needs in partnership. We also offer separate carers needs assessments and related support.
The team provides intensive support using a team approach to severely mentally ill people who are ‘difficult to engage’ in more traditional services, many of who will have a forensic history or a dual diagnosis and often lead quite chaotic lifestyles.
Care and support is offered in our service users’ own homes or some other community setting, at times suited to them. Workers are involved in direct delivery of practical support, care co-ordination and advocacy, as well as therapeutic input.
