The Assertive Outreach Team is a multi-disciplinary team and works to promote social inclusion for service users through developing and maintaining long term supportive and therapeutic relationships with people, maximising their strengths and abilities while acknowledging the limitations and difficulties caused by their disorder; and working alongside service users and their carers to identify and meet their needs in a partnership approach.
The Team provides intensive support using a team approach to people with a severe and enduring mental health problems who have a revolving door hospital admission history, and are ‘difficult to engage’ in more traditional services. This could be due to previous negative experiences, severity of mental health state, not acknowledging a need for statutory intervention to monitor mental health state, offer treatment options, and/ or manage risks. Others may choose not to engage due to life style choices, which may include the use of drugs/ alcohol. The team adopts a team approach in deliverying care packages and to manage risks, and work closely with other agencies including police, probation, housing agencies, drug & alcohol services.
