Care programme approach and care pathways

All assertive outreach service users will normally be subject to the care programme approach (CAP) due to the complexity of individual needs.

Experience suggests that individuals who traditionally don’t engage well with services dislike frequent formal reviews. However these are necessary due to the complexity of service user needs and the nature of assertive outreach interventions.
