The Community Mental Health Team (CMHT) offers advice and support to older people in the community with mental health problems.
Once referred to the service a community mental health nurse, memory service nurse, social worker, an occupational therapist (OT) or psychologist may see you, or you may be referred to another team within the service.
Community mental health nurse/memory service nurse
A nurse may visit you at home and assess your mental health. They will then coordinate care and treatment.
Occupational therapists (OTs) look at all the aspects of daily life a person may be having difficulty with. From self-care, to cooking, to accessing meaningful activities.
Social worker form a link between social services and the NHS. A social worker may visit you at home to help sort out problems with care, health, housing or benefits.
A clinical psychologist works with individuals with a range of mental health/behavioural problems or trauma histories, using a range of talking therapies.
Psychology is the study of mind/behaviour, and psychologists use theory and information from the patient, or families and carers to make sense of their difficulties.
This assessment then leads us to make choices about how to help the patient. All work is collaborative with patient and family.
