This service supports older people with mental health problems at home and in clinic based settings, helping people to stay at home and manage their difficulties.
Our service receives referrals from our local general practitioners (GPs).
We are an integrated team which provides assessment, treatment interventions and support for people with mental health problems and their carers.
We have nurses, doctors, social workers, occupational therapists, physiotherapists, psychologists and support and administrative staff in our team.
To access our service please see your GP to discuss a referral to us.
We are open on Monday – Friday 9am – 5pm (except bank holidays).

During office hours:
If you, or someone you know has concerns about their immediate health and you or they are already known to our services then please use the following contact methods to get the help you need:
1. Call your care coordinator or lead professional – reception (01724) 382000. If they are not available, the receptionist will guide you to a suitable alternative individual to deal with your concerns.

Out of office hours:
If you have a personal budget you may have an allocated amount to use for emergency respite.
If you feel the situation is deteriorating please advise your care team as soon as possible so contingency planning can be put in place (e.g. additional help for a short period of time) for out of hours emergencies.
If you attend an accident and emergency department there are mental health staff available, who can be called on to respond to mental health emergencies.
