Assessment and formulation
The Assessment and Formulation team conducts initial assessments for individuals to be referred to specialised mental health services in Rotherham.
How we can help
The service collaborates with you to create an initial care plan, identifying immediate actions for patients and their care worker. Your care worker gathers information about your needs during the initial appointment and determines the appropriate services to meet those needs.
How to get help
Referrals to this service are made through your GP, health or social care worker, community nurse or care manager.
