The Support Therapy and Reablement team (STaR) comprises mental health professionals collaborating with the broader mental health service for older adults. Our focus is on enhancing both mental and physical wellbeing.
We strive for a smooth transition towards independence and engagement with suitable services.
How we can help
Our team collaborates with you to devise a personalised care plan centred around your mental wellbeing.
We offer short-term group sessions and one-on-one meetings in community settings, aiding your reconnection with the local community by participating in groups and meeting new individuals.
How to get help
Referrals to our service typically come from mental health professionals you might have previously encountered. These may include community nurses, occupational therapists, physiotherapists, and consultant psychiatrists involved in your care.
If you and these professionals believe that you could benefit from the support and services provided by our team for your mental health, they will make a referral to the STaR team.
