The Enhanced Care Home team provides specialised support encompassing mental health, physical needs, therapy, and fall prevention.
How we can help
We conduct thorough holistic assessments for residents, aiding care homes in tailoring individual care plans to address specific health needs.
Our team provides specialised support through community mental health nurses, offering diagnosis, treatment, and proactive management. We also assist individuals who have experienced a fall.
Residents have access to physiotherapy and occupational therapy services to enhance their overall well-being, focusing on activities of daily living, exercise, posture, mobility, and more.
Non-urgent advice: How to get help
Referrals for permanent care home residents can be initiated through the single point of access (SPA).
