The Children’s Community Nursing team consists of skilled children’s nurses offering care and support to children, young people, and families in the community.
How we can help
We collaborate with other professionals to assist children with the following:
- short term conditions
- long term conditions
- disabilities and complex health needs
- life limiting and life-threatening conditions including those requiring end-of-life care
Non-urgent advice: How to get help
We follow a referral process and welcome referrals from GPs, hospital consultants and other health professionals.
