Our nurses can help you understand your condition, manage your medications, monitor your blood glucose levels and make lifestyle changes that support your long-term health and wellbeing.
We can signpost you to local diabetes support groups and educational resources, helping you connect with others and access information to support your long-term health and wellbeing.
Non-urgent advice: How to get help
This service accepts referrals for people with complex diabetes needs who would benefit from assessment and advice from a diabetes specialist nurse.
Referrals are made through your GP or healthcare professional.
Once we receive your referral, you will be offered a clinic appointment with a diabetes specialist nurse to discuss your needs and agree a personalised care plan.
