The Specialist Community Forensic team (SCFT) closely collaborates with patients affected by mental illness or personality disorder, who have either offended or display a potential to offend, posing risks to themselves or others.


How we can help

Our primary focus involves supporting safe transitions from hospitals to the community, ensuring patients receive appropriate care in suitable settings while prioritising personal and public safety.

Our care and support are tailored to individual needs, involving comprehensive risk assessments and management strategies to ensure the safety of the individual and those supporting them.

For patients in hospital, we provide in-reach support and ensure safe and well-coordinated discharge planning.

We offer consultation, support, and collaboration with partner agencies. In the community, we provide guidance on available options, risk management strategies, and necessary adjustments.

Additionally, we provide offence-specific intervention training through our broader team, collaborating with internal and external agencies to enhance client care and staff confidence when working with forensic populations.
