The Neuro Rehabilitation Outreach team offers an integrated rehabilitation service catering to adults in Doncaster and Rotherham diagnosed with neurological conditions.
Our aim is to assist them in achieving their optimal levels of:
- physical function
- cognitive function
- communicative function
- social function
- behavioural function
How we can help
Collaborating across health and social care, we focus on supporting individuals with complex needs. This involves joint home visits to assess both the patient’s and carer’s requirements.
Our treatment is tailored to the specific needs of our patients and is usually administered in their home, a hospital setting, or local facilities to ensure the most suitable environment.
