Cardiac rehabilitation
Cardiac rehabilitation is a program designed for individuals post a cardiac event, surgery, or diagnosis. Our team’s goal is to assist you in returning to a fulfilling and active life while instilling confidence in managing your condition for the long term.
How we can help
Working closely with our specialist cardiac team, we provide comprehensive education, lifestyle guidance, counselling, and structured physical activity and exercise to aid in recovery and lower the risk of further cardiac events.
Upon referral, your nurse will discuss your diagnosis, medications, and lifestyle with you. Following this, your physiotherapist will offer more tailored advice on exercise and physical activity.
How to get help
Referrals to our service can be made through your GP, healthcare or social care worker, community nurse, or care manager.
