Cardiac rehabilitation

Cardiac rehabilitation is a service that creates a plan for people after a heart event, surgery, or diagnosis. Our team helps you return to an active life and feel confident about managing your heart condition in the future.

How we can help

After your heart event, a cardiac nurse will offer you an appointment at clinic or at home if needed.

We can offer a wide range of physiotherapy options, including:

  • group exercise and education sessions
  • one-to-one sessions
  • home visits
  • walking programmes
  • adapted Tai Chi and relaxation sessions
  • home exercise plans
  • self-help and instructional videos
  • phone support or remote monitoring if you prefer

If you have a new diagnosis of left ventricular systolic dysfunction (LVSD) or arrhythmia after your cardiac event, we can refer you to the heart failure and arrhythmia service.

If you need extra emotional support, we can refer you to Talking Therapies.

Our team of specialist nurses and physiotherapists work closely with your GP and cardiologists to create a personalised plan. The aim of the personalised plan is to help you:

  • understand your heart condition
  • learn about healthy lifestyle choices, including exercise, diet, weight management, and stopping smoking
  • make sure your medication is right and symptoms are under control
  • support your emotional wellbeing
  • reduce the risk of future heart problems and improve your quality of life

Our service is checked and approved by the National Audit of Cardiac Rehabilitation (NACR). This means your rehabilitation plan follows trusted guidelines, your progress is measured, and everything is checked often. This makes sure your recovery program is safe, high quality, and helps you get the best results.

Non-urgent advice: How to get help

Patients are usually referred to us after certain heart events or treatments, such as:

  • a heart attack (myocardial infarction)
  • planned procedures such as a percutaneous coronary intervention (PCI)
  • heart surgery, including valve repairs or replacements and coronary artery bypass graft (CABG)
  • other cardiac events or surgeries

Most referrals come from hospitals after an emergency admission, but you can also be referred by your GP or cardiologist.

We also accept referrals for physiotherapy through the heart failure and arrhythmia service if cardiac rehabilitation would help your recovery.
