Therapy hub

The therapy hub comprises physiotherapists, occupational therapists, and therapy assistants who receive referrals from RDASH colleagues and Doncaster GP’s, working across various home first pathways such as:

• occupational therapy
• reablement
• urgent community response
• virtual ward

How we can help

The primary goal is to enhance and sustain a patient’s functional abilities and independence within their home setting to improve their overall quality of life.

The team conducts person-centred comprehensive assessments, delivers interventions, and makes onward referrals if required.

How to get help

Referrals are facilitated through the single point of access (SPA) using the appropriate referral form.

There are exclusions to referrals for certain diagnoses. Patients with neurological conditions, learning disabilities, or those who have had a recent stroke should be referred to the specialist services available.
